Thomas (Al Pacino) is a US expatriate in Italy, Napels who works for the infamous Camorra organized crime. He and his schizoprehnic brother Ruby, who also suffers from an obsessive-compulsive disorder, run a heroine-business there, which they sell to not only members of the Camorra family, but also to other criminal gangsters who are very low-members of the organization.
Ruby is older then Thomas who often annoys the agressive Thomas, which results that Thomas often leaves Ruby in an alley in hope someone would kill him.
